Why Acne Leaves Dark Spots: Understanding Post-Inflammatory Hyperpigmentation
By Dr. Manaswita Roy
The pimple may be gone, but the mark can stay for months. Here is why that happens and what a dermatologist-led plan can do differently.
A flat brown, grey, or darker patch where an acne spot used to be is often not an acne scar. It may be post-inflammatory hyperpigmentation (PIH)—a change in skin colour that appears after inflammation settles. PIH is particularly common in people with medium to darker skin tones and can last much longer than the original breakout.
Why does acne leave a dark mark?
Acne triggers inflammation in the skin. During healing, pigment-producing cells can become more active, leaving extra melanin in the affected area. The result is a flat dark spot that is different from a depressed or raised acne scar.
PIH versus an acne scar: what is the difference?
- PIH: a flat change in colour. The skin surface remains smooth.
- Atrophic scar: a depression or indentation that changes the skin texture.
- Raised scar: thicker tissue that sits above the surrounding skin.
You can have more than one type of mark at the same time, so a proper skin examination can be useful when the appearance is unclear.
The first treatment step is controlling new acne
It is difficult to clear pigmentation while new inflamed pimples continue to create fresh pigment. Dermatologists often treat the acne and the dark spots together, with a plan suited to the type and severity of acne.
Sun protection matters more than most people expect
UV and visible light can contribute to ongoing darkening. Daily broad-spectrum sunscreen is therefore a foundation of pigmentation care. For some people with darker skin tones, tinted sunscreen with iron oxides may provide added protection against visible light.
What treatments may be considered?
Treatment depends on how deep and extensive the pigmentation is and what is causing it. A dermatologist may consider topical ingredients, prescription medicines, chemical peels, or selected laser-based procedures. The right sequence matters because overly aggressive treatment can itself cause irritation and more pigmentation.
Avoid the "more is better" trap. Scrubbing, repeatedly changing strong acids, or using multiple irritating actives at once can damage the skin barrier and make the colour change harder to control.
Why skin of colour needs a careful approach
Procedures such as chemical peels and lasers can be useful, but they need thoughtful patient selection and settings. The American Academy of Dermatology specifically advises people with darker skin tones to consult a dermatologist before using peels, microdermabrasion, or laser therapy for acne or its marks.
When should you get professional advice?
- Dark spots keep appearing with every breakout.
- Acne remains inflamed, painful, or widespread.
- You have tried over-the-counter products for several weeks without meaningful improvement.
- You have both dark marks and visible texture changes or scars.
- You are considering a peel or laser and want to reduce the risk of post-treatment pigmentation.
A realistic goal
PIH usually improves gradually rather than overnight. The most useful strategy is often a combination of acne control, pigment-targeted treatment, gentle skincare, and consistent sun protection. Your dermatologist can adjust the plan as the skin responds rather than adding more products every few days.
When acne and pigmentation are treated together, the aim is not only to fade the marks you already have, but also to reduce the cycle that creates new ones.
